My skilled poster presentation
I recently had the chance to create an expert poster for the Comprehensive Primary Care Plus (CPC+) conference in Baltimore, Maryland. CPC+ is a national advanced primary care home model that goals to strengthen primary care through regional multipayer payment reform and remodeling care delivery. Care delivery provides clinics with the infrastructure to offer higher care, leading to a healthier patient population.
For my poster, I selected the subject to focus on two unique bands I work with. As a Nurse Care Coordinator (NCC), I discover high-risk and clinically complex patients in the first care outpatient office setting. In my position, I actually have the chance to work with behavioral health consultants (BHC) and community social employees (CSW) to assist improve the care I provide to those patients and their relations.
It was an motion research poster by which I described the role I play and the way BHCs and CSWs help improve patient care. This is a subject I’m captivated with and need to share with others. Remember that when preparing your personal skilled poster, select a subject that you just are captivated with, that you wish to learn more about and share your discoveries with others.
The conference itself was an incredible experience! I had the chance to attach and network with all healthcare professionals – from doctors, to nurses, to pharmacists and plenty of others. It was interesting to learn what other health systems outside of Pennsylvania were doing in the first care space. There were keynote speakers and plenty of breakout sessions where I used to be in a position to learn more about topics that interested me, reminiscent of:
- It’s time to integrate care management
- Redesigning take care of seriously sick patients
- Delivering care through revolutionary primary care practices
- Work outside your practice to satisfy community health needs.
Overall, presenting my poster was an incredible opportunity to attach with others within the nursing industry, network, and study many other interesting topics on the conference. I highly recommend everyone to try their hand at creating an expert poster to present at a conference. There are so many great nursing conferences – from local to international – and when you attend one, you will be hooked and need to attend more! This is an incredible opportunity to showcase your knowledge and fervour a few topic you enjoy. Sharing it with others is incredibly satisfying!
