I do know, I do know, one other care plan… but this one is for YOU! Use this quick care plan to be sure that you are on the right track to achieving your goals!

  • Review my medical records and make a listing of really useful screenings and vaccinations based on my age and medical and family history.
  • Update my calendar with work schedule and upcoming social events. Also plan “me time”!
  • Consider how I best deal with stress. Is this a yoga class? Reading? Being outside? Find activities that meet my stress needs.
